Treatment of Haematoma's?

One of my horses has a fairly large Haematoma on its tummy after trying to climb out of the horsebox over the partition

Its about the size of two large tennis balls.

Do I have to get the vet to lance it, or can they disperse with time?
 
My boy had to have his heamotema lanced and drained because the bacterica was going around his body and making him poorly i think i left it about a week or so coz it started small and got bigger he needed a week of anti bs and pain killer because i had to flush it every day!
 
2 i can think of we have just gave antibiotics and bute and cold hosed with gentle walking to disperse it, but if it didnt go done then I would ring the vet.
 
i have had 2 horses with haematomas (one reared up and fell over onto my leg and the other jumped out of his stable and caught his stifle!).

both were treated with hot and cold compresses.

the one who had it on it's side went down after about 3 weeks.

the one with it on the stifle had it for 2 months or so- the vet checked after about 6 weeks (horse was having jabs) and he said to leave it as it wasn't affecting the horse.
his was huge though (more the size of 4 or 5 tennis balls).
