Underweight horse- what to feed?

My 17 year old retired MW cob got bad laminitis last winter and from then he went on restricted turnout then gradually built it up untill all day with his green guard muzzle on and he has been doing well and was looking really healthy (ribs easily felt) but in the last couple of months he started to look poor where you can see all ribs. Was advised by vet to contact nutrisionist about what to feed and was advised to introduce high fibre nuts with his plain chaff and he now also gets 2 large haynets a night but was told to leave his grazing mask on, this was about 4 weeks ago and there has been no improvement so far!
He has had teeth done recently only a couple of months ago but have got dentist coming out to check him over again, he had all sorts of tests done in spring and appeared to be healthy, don't know if further ones should be done?


How long would you expect it take before you see some slight improvement?
