Unusual lameness

My boy was lame a month ago and vet said it was concussion on his front foot as he has flat feet and advised his shoes were taken off and he was rested and given bute. Was still the same after 2weeks so i phoned vet who was coming out to nerve block. Fortunately he came sound before the vet visited. He has been ok and been going out with friends in the field until today, I went to get him in and he kept stumbling every few strides. There is no swelling or heat. He looks sound in walk apart from his stumbling every few strides but is clearly lame in trot.
It doesn't seem to be painful to him and he is still being his cheeky self.
Obviously, will be getting vet but just wondering if anyone knows what it may be or had anything similar as my vet really has no clue without doing nerve blocks/scans etc.

if your vet has no clue about nerve blocks then get an equine vet out to assess. it is obviously painful to the horse as he is lame.
 
Ofcourseyoucan, I meant the vet has no clue about his lameness without nerveblocking which is what he is going to have to do.
Classicalfan, do u say that it's not his legs as his lameness is intermittent?
I will defiantly have a feel all over him tomorrow and see if he seems sore anywhere.
 
My horse was intermittently lame before shoeing and came sound after. He would stumble and be lame in trot. Had X Rays done which showed Ring bone in both front coffin joints. He's sound again now and on no bute.
 
Just something else simple - as the ground has hardened out again - is it more rutted coming in from the field now? if your horse has flat feet could he still be feeling it? I had a TB who would suffer easily from bruised soles. Weird though it sounds he would be completely sound trotted on a hard flat surface (and circled) and hopping lame in the school - uneven surface touching his frogs. He ended up having pads on and was fine.
 
Thanks for the advice. I've had the vet today who has confirmed the lameness is coming from both feet. He is due again on wed to xray his feet. Arthritis of coffin joints, navicular and insulin resistant lami were suggested by vet. He is showing no reaction to the hoof testers and has no bruising of the soles or hemoraging of the White line.
 
UPDATE:
Had xrays today... No signs of lami or anything. Only unusual thing was a abnormal bone growth on his short pastern right by the joint. Vet thinks it maybe interfering with the joint so he has got to do a joint block :(
