Urgent help needed!! Where do you get medical cards for armbands?

Usually from british eventing,but if you need one for tomorrow just copy info from someone who has got one,its just info like name age next of kin doctors name and number and any allergies
 
Will it matter that its not an 'official card'? Only it states on my entry form that I have to have one and I just didn't want to travel miles to the event to find out I'm not allowed to enter!!

They dont vene look at the "card" aslong as you have the armband and some details in, it wont be a prob. I know a few people that have forgot them BE, borrowed someone elese armband and then written details on a bit of card!

I just wrote my details on a bit of paper and stuffed it in, no-one checked so guess it must be ok
