Very sick pony - ideas on what it could be?

Our pony has got some sort of mystery illness. The vet has just brushed it off (as she has previously) but there are no other decent vets in the area. We are going to try and get him an emergency appointment at a clinic in the next city on mondah

He was stiff behind and spiking a temperature yesterday of around 40. Last time this happened we eventually found out he had a liver infection. Today, his right leg is swollen from the hoof to the hip and he can't walk on it. There's no heat in it, just swelling. The Hindquarters are twitching too.

He's been very sick multiple times since he had African horse sickness but has always got better and been fine for months or years at a time but honestly we're a little worried he might not improve this time :(

Any thoughts or ideas would be great.
 
It sounds like Lymphangitis or cellulitis but I would expect the limb to be hot not just very swollen, his immune system may still be weak from the AHS or another flare up of the liver problem combined with an infection in that leg, even a tiny scratch can become infected if they are struggling in some way, I hope you can get him the help he requires, it cannot be easy living where veterinary care is so limited.
 
Has he been vaccinated for AHS recently? There have been a number of adverse reactions with recent batches of the vaccine. Otherwise, have you done a good check of legs, including between the hind legs. We had a mare who presented similarly and then started showing some neurological symptons. She had been bitten by a cobra. We presume she must have been caught whilst rolling as the bite was high up and not immediately visible.

I don't know whats wrong with your horse but just wanted to say I hope you manage to get it sorted. You must be desperately worried and to not have any support from a decent vet must be very frigtening for you. You have my sympathies.
