Mud fever. Would probably slap on some Hoof to Heel or MSM cream on to soften and get off the last few scab (don't pick, just rub in cream and the sluff off).

I keep putting cream on mine till there is no raw bits left (3-4 days normally). I also put a barrier cream on Hoof to Heel or Sudocream if I'm turning (if not too muddy) and the hair hasn't quite grown back (which takes a few weeks). I have found that mine hasn't had mud fever since I've started feeding Echinacea through the winter (this is my 2nd year), its supposed to help boost the immune system.

Looks just like what my cobs got, which started as a graze/cut then spread into a crack.
I took Piebaldsparkles advice and bought some hoof to heel , seems to be the best thing ive bought.
its lanolin based it stays in place really well and after only 2 days of using it the scab has come away!
As for your pony thats what i would use, and not wash it, thats the mistake i made and my ponies had a crack for nearly a month, let us know how how she does. When the scabs come off and the areas healed i would use a barrier cream until all the hairs grown back.
Ive also started my cob on an echinacea based product Propell, to help boost his immune system.
Also in past experience the swelling may not go down until the last packet or so of antibiotics (norodine) so try not to panic if it doesnt go down straight away
