First off, fortunately it's not my horse that's lame but one of the other liveries at the yard.

When taken to be ridden horse is sound (albeit a bit stiff) in walk, reasonably sound although again a bit stiff in trot on right rein and very lame in trot on left rein - issues seems to be around back left leg around hock area.

This horse had a lameness issue on the same leg around 4 weeks ago which owner treated with a few days box rest and some bute (no vet involved)

Horse has had surgery on both back legs for bone spavins in the past.

What would you do with this horse - keep it stabled, turn it out to stop any stiffening, call the vet or wait and see how it goes?

Vet immediately. I'd want the vet out first thing tomorrow morning and then you don't have to make a decision about turning out or not.
