why does she decide to fling her head and make contact with my face

Hi, Hope someone can help me. my horse is just used for hacking and general pleasure riding. when i got her her head carriage was very high. this has improved. The only problem is we can be canterin, galloping along and for no reason she will start to through her head right up for a few strides then stop. she is a cob cross and I found a running martingale did not help. her back is fine and her saddle was made from scratch for her. I have been lucky so far and only recieved a bruised chin. I don't have a school available has anyone got any suggestions of training aids or problems that could cause her to do this. teeth checks are upto date.
 
i wondent go strapping her down just yet, i would get pro help first from either a riding instucter or someone you no that will help you.
 
I have a dutch gag in. I know this is ment to have a head raising effect but it is the only thing i have control with when out in company going over the fields. She goes relaxed in it the rest of the time no head shaking. I feel she is happy with it. She has a tick tough and fat lips. I have tried a hanging cheek but I may as well of been riding her with head collar on.
