You should discuss with your vet but the 5 stage vetting is a standard procedure so you can largely rely on it covering what it is meant to - google or the vet can tell you the procedure. The vet will want to know what you plan to do with the horse and at what level - be clear and honest about this as it will help the vet assess his/her findings ie don't make out you need a high level competition horse when you just want a happy hack and do a local show occasionally. Also find out what your insurance company will require is done. The vet will also likely ask whether you want blood taken (yes you do), any x-rays (you may or may not, discuss pros and cons with the vet but they will increase the cost significantly and my not help you decide). Also is there anything the yard you are planning on keeping it at requires for a new horse? Some require a strangles test for example - if yours does, get this done too if the horse passes. Also be aware they do not "pass" or "fail" as such, although the vet will likely make a recommendation but the findings need to be discussed with you and you need to decide what you can live with. No horse is perfect.

Your vet should be able to cover all bases without you requesting anything specific, in terms of soundness, temperament etc.

I would just be crystal clear with your vet about what you wish to do with the horse as this will influence a pass or fail.

This.

You can, however, ask them to particularly check anything you have a concern over.

And yes - attend.

I have brought up things that concerned me on my recent vettings in a chat over the phone with the vet prior to the day. For example, lack of muscle tone over the quarters and back on one, another one did the odd trip which I flagged...
