Advice on Sheath beans

Hi everyone, I am looking for a bit of advice.
Back in October I had the vet out to remove a sheath bean, they removed one high up that was causing my horse discomfort. He was very sluggish before it was removed, now he’s like red rum but I am concerned there is still one at the very tip (just above urethra) as I feel he still may be in some discomfort. However, I don’t feel I’m knowledge enough to know for definite. How can you tell for certain there is one? I also don’t want to approach the vet without knowing for sure!

There was a company that came to our yard that specialises in cleaning geldings sheaths. Might be cheaper than a vet call. I'm sure there are many that do this all up and down the country.
