BHS Stage 1 application form

Hi Just two really basic questions about the BHS Stage 1 application form.

Firstly, can the witness be anyone?

And secondly, when paying by credit card do I just write the card details on back of form as there is no specified section for this?

Not too sure but I'd just give the BHS a ring, far easier! Got my stage 1 on Tuesday eek!

Most secure thing to do is phone the details through as well
