I don't believe feed is the issue or supplements she has been on the same feed etc for the last 3 months+ which has been advised for her by Top spec?

I believe that TopSpec, even though it is advertised as cereal free does indeed contain cereals. As I was reading your OP, I was thinking "Food". I was reminded of the WelshxTB mare I used to own who was extremely sensitive and apparently unpredictable, although when we knew it was food related we could predict her behaviour and related it to when she had last eaten. There were times when she seemed oblivious to what was going on around her/where she was - very dangerous!Please for your horse's safety and your own take her off all feed and supplements, except grass/hay for 3 weeks and see if there is any improvement.
