Bridle falling off

I recently fully clipped my ISH for the first time since owning her and since then her bridle keeps falling off when I’m riding her.
We’ll be walking down the road and she’ll shake her head (not aggressively at all) and it’ll just drop off over her ears.
I’ve had a saddle/bridle fitter out recently to get them to see if it is the correct fit and she said there was absolutely nothing wrong so couldn’t understand why it keeps flying off.
She does also wear as ear bonnet, but that hasn’t affected it staying on in the past.
Any suggestions as to why this is happening and what I can do to stop it?
 
I had a pony many years ago, which the bridle would fall off from and for no apparent reason.
I used to do a plait in the mane just behind the ears then loop a boot lace through the plait, inbetween the ears, loop round the top of the head piece, and tie.
 
Does it still happen with Micklem bridles and ear bonnets? It would be a lot harder to dislodge, I would think?

I can’t use an ear bonnet with Polly because whenever she shakes her head, the bridle comes off. It doesn’t happen with Millie so must be to do with the shape of Polly’s head or something.

It only happens with Louis if I use the acoustic ears, it's doesn't happen with Arabi so maybe it is that!
 
My last memory of ear bonnets in the 90’s was that they had a string to tie under the head, like a throatlash - literally, bonnet strings! They weren’t particularly fashionable looking things, so I suppose ear bonnets may not have string anymore? Seems redundant, if they fly off now! If it didn’t have a string, I’d plait the bridle into the forelock.
