can changing saddle help hip pain?

Shortening stirrups is the easiest way to ease hips, it just isn't always right for that individual on that horse. It is why super wide dressage saddles don't sell in big numbers, as so many riders have to ride a little shorter on a very round ribcage.
 
Not sure if this has already been mentioned but can you not buy a gel seat saver pad like acavallo pictured below? When I had my slipped disc it was an absolute life saver on Bailey as she had the most amazing canter with a very big action behind and it really used to irritate my disc when I came back into riding.

It might help you as it absorbs some of the impact. Its the same stuff that is in the seats of wheelchairs. You could also take anti-inflammatories like ibruprofen and paracetamol but think carefully if the doctor prescribes you co-codamol as its very addictive.

Most seat savers make the saddle slightly wider rather than narrower (except the aforementioned HM Hip Saver), and can even leave a gap below the skirts under your thigh which may not sound an issue but it really is. They can help with impact but Thinline and Block the Shock are made of more proven materials.
