So I took my stage 2 yesterday (I won't say where) and got through to jump. The horse in my jumping took off and jumped over a foot clear of the double. The second time down the double he did the same thing, then proceeded to buck me across the field, eventually throwing me off. The examiner demanded the horse was removed and then They allowed me to continue on another horse and that went fone. Then the third horse was a little hard to stop (turns out he's normally jumped in a Pelham with roundings) I feel like I've failed due to this (all of the horses refused jumps are no riders went clear) I would like a copy of my accident report to see what they said as I was asked to sign an empty form. If I've failed I will be fuming!
 
I think it will be a judgement call as to what happened in the actual jumping, a witness/assessor may have a different view than yours. Once a horse has bucked a rider off, whatever the reason, it would have been taken out of the exam. That is best practice, and not necessarily an admission of the horse being "dangerous" per-se.

Many horses could be upset by unusual riding, and could take a fright. The horse was replaced with another which went well. In any case you are judged on how you deal with a situation, and people who have fallen off have passed if there was a reason.

The accident form incident is another matter. I personally would not have signed a blank form, that is not correct procedure. You can request a copy of this.

I do hope that you are not hurt, and that your exam result is a success.

It is always very upsetting when something untoward happens on your assessment day. I took a fall on one of mine and, due to hitting my head very hard, was not permitted to continue. I was upset at the time but in hindsight it was absolutely the right decision that was taken. It is like your driving test, circumstances on the day can have an effect. That is just like real life driving, just like horses can vary in every day life.

If you are concerned about the accident report, ask the education department at BHS HQ.
If you feel you were treated unfairly in your assessment, there is a procedure which can be followed.

Red-1 is correct, many people have been successful in the assessments as, despite falling off, they were of the required standard. Others will not have been due to not meeting the standard.
 
Don't assume you will have failed, I know one Stage 3 candidate that got bucked off 3 times on the xc and still passed - I think she would have been stopped from remounting except that she jumped up and on really quickly after each fall!
 
You need to deal with the signing of the form on Monday if not before. The accident form must be completed by the riding school or company first, it details what happened and then you sign it to agree that the account is factual, truthful and correct.

Anything could be put on an empty form and if, tomorrow you find you have a serious injury and the whole situation becomes a legal battle you have an enormous problem on your hands. Were there any witnesses to you signed the blank form ?
 
I agree with the others - you need to deal with the blank form pronto. You could even say in hindsight you'd not have signed a blank form but after the events that took place it was the least of your concerns at the time!
If the examiners felt that you rode to the required standard even including the fall (anyone can fall off after all depending on the horse and circumstances) then you will pass. It's how you deal with it as much as anything. In my stage 3 there were several fails in the riding with no falls because the examiners just felt the riders weren't balanced enough (they weren't allowed to continue to jumping if failed flat or continue to x-country jumping if failed the showjumping). I was quite shocked at the poor balance of some of the riders and the examiners tried to help by asking (inviting?) the riders if they wanted to adjust their stirrups etc. The riders were all complaining afterwards as felt it was unfair.
