Hiya all,
I would be very greatfull if you could give me some advice!
Bob the Cob injured his deep digital flexor tendon back at the beggining of May, we think he slipped in the field! Vet said the tears where small and prognosis was good (80% full recovery) He had a small swelling but no heat/lameness. He had 6 weeks box rest, 6 weeks pen/box rest with inhand walking increasing in time every week then was rescanned. He appeared to be healing well and the tears where filling in (although difficult to scan because of his thick cob skin :P) and the swelling had gone completely which the vet had advised may stay forever! Vet advised 3 months field rest to allow his tendon to heal further and just to give him time!
Well the three months is nearly up and the plan was to bring him slowly back into work. However friday evening he came in from the field with the area around the tendon quite swollen (very small amount of heat/ no lameness) I would say it was more swollen than it ever was initially but a different kind of swelling! Before it looked almost windgall like (vet initially diagnosed as that) where as this was over a larger area and was softer. He was quite warm so I suspect he had been running around in the field. Iv been bandaging/ice tight/cooling gel, basically everything to get the swelling down again and tonight (2 days) it was nearly back to normal but still not how its been for the past 3 months.
But! the paranoid me is worrying he has redamaged it and I really dont know what to do!
1) Rescan to check? (and cost me even more when iv just started uni so feeling a bit poor!)
2) rest for a few more weeks continuing ice/banadge etc
3) continue with plan to bring back into work and assume that swelling is due to tendon being stretched alot more than it has for 6 months and that is just the response seen as surely after 6 months it must have healed by now?

Annndd finally if anybody has a general plan for bringing back into work they could tell me I would be very greatfull! (as in how long to begin walking for each week etc as I agree with the whole 6 weeks walking to stengthen things up first!)

Wow that was an essay, sorry!

Thanks :)

Tendon damage tends to be at the micro-tear level (unless you're really unlucky). The damage is filled with scar tssue and then the body gradually remodels, replacing the lumpy scar tissue with tendon cells. There usually is some scarring left. This whole process takes about 15 months. Any further damage is usually above or below the scar tissue, hence the damaged area increases with each injury and the recovery period is likewise longer.

In our humble opinion vet advice is barmy which is why we end up with so many tendon injuries sent to us. We never box rest and they are out 24/7 to keep everything moving and supple (and to stop them going out of their heads with frustration). The more you restrict movement the more likely it is that the tendon will tear again because the scar tissue is 'fixing' the static damage and needs to repair the 'working' site.

i would rescan. my mare did her ddft in August last year and had 8 months on restricted turnout. i started walk work at 5 mins 2 x daily mainly as this was what vet advised for a different horse i own returning from injury but i did it slowly as my mare is 19. we increased by 5 mins a week until we were up to an hour then introduced trot. she is sound and feeling great however i have increased the size of her turnout only recently and have a fear of her doing it again... good luck with yours.
