Dog peeing on our bed - help!

We have a rescue jrtx (poss with a spaniel) about 4 years old. She's very good natured, abit submissive but a happy dog. We both work but my OH works shifts. We have a schnauzer x also. The jrtx took quite a while to house train. When she was crated, she used to wee in there. She is now pretty good, but on occasions she wees on her cushion in the lounge, and tonight I found her on our bed, and she had peed on it. She sleeps on the landing, not in our bedroom. Despite the fact she had had the run of going outside. A few months she pooed on the table! There has been no alteration in her routine and she gets plenty of walks. She is overactive and no matter how long a walk she has she's never tired. She also,has a very short attention span. Any ideas what to do, please?
 
Keep her out of all the areas you don't want her to pee in, IE keep the doors shut and be strict about it.

'Her' cushion needs to be very thoroughly washed so it does not carry any of the pee smell.
As for the crate, make sure it is not too big - all the dog needs to do is stand, sit, turn around, it's not meant to be an indoor kennel.

Also get her checked out at the vet's for a UTI. Keep an eye on her water intake, if she takes a big drink, then watch her like a hawk and when she looks as if she's about to go, sniffing, circling, whatever, then straight outside.
