it can be a sign that there is something amiss.
or some if they are weak behind and let them slop around in the forehand they will trip.

It's a lot more effort to dip the shoulder for a stride than to take a normal stride so I doubt it is laziness

That sounds more like the horse is not landing correctly on one foot especially if it is always the same one, the dropping feel is not what most would think of as dropping the shoulder which tends to be a faster evasive action often combined with a buck to unseat the rider, I have seen it in a few and it looks as if they either are avoiding landing heel first or that their knee is not wanting to straighten, lock, for the odd stride, it can be laziness but it can also be an early sign of something being wrong physically so definitely worth getting it videoed or an experienced pair of eyes on the ground, also consider the surface it happens on it could simply be an arena with bits they catch their toe in at times, if it is on a level road I would be more inclined to think soundness issues.
