Ear Problem? Or something else?

Just been to visit my horse and I noticed she's holding her left ear flat/letting it hang and was shaking her head a lot, seemed very irritated by it. Googled ear infections and it seems to be saying they are rare. Could this be something else more common? Has she maybe just got a bit of something stuck in it or itchy mites? I tried looking down it but she was a bit precious about it. Wondering if I should arrange a vet visit or if it could be something simple/obvious?

eta: was also wondering if it could be tooth irritation on that side of the face or something rather than the ear itself?
 
My horse wandered round for an afternoon a few years ago with a 'broken' ear at half mast when I accidentally got water down it with the hose pipe! I was horrified that I'd upset her so much but she got over it, perhaps it was something similar :)
