Exercises to get horse using shoulder

Ive recently taken a horse out on loan whose proving to be fantastic, hes 17 years old but when he was 12 he had a shoulder operation quite high up in shoulder. After surgery he was brought back into very carefully and ended up competing very successfully at dressage up to elementary level. 2 years ago due to owners circumstances he stopped competing and was just used for hacking. Ive just taken him on and now need to get him using that shoulder again as his left shoulder is significantly smaller than his right and hes learnt a way of going without using it. Hes also developed a huge muscle at the top of his left leg and my physio said hes using this muscle to push his leg though and forward rather than his shoulder. My physio has told me to do lots of raised poles and given me a couple of ground exercises to use but I just wondered if anyone had any other ideas at all. I should also mention that I took him out to dressage last week and got 68.8 % in our prelim test so its not massively affecting him but the judges comments did mention ' not always consistent in trot'
