Gelding peeing across his tummy

Ok.. I am looking after an elderly arab. Since this winter. We sold him many many years ago, he was my daughter’s first little horse. Sadly his owner (who I kept in touch with) suddenly passed away at a young age. And I was contacted by the yard to see if I could possibly take him. He’s got Cushings. Currently on half tablet a day. And very dropped rear fetlocks. But he’s only a five year in his mind. Which is sad. And still has a good skip and jump when he canters around the field. Anyway.. he’s got a big swollen scrotem. Cold. Not hot. He has apparently had it for years and Cushings can be a side effect. And previous owner vet wasn’t too worried. Plus I suspect heart. So .. yesterday he had filled up the inside of his sweetitch rug with pee. Which would mean his Willy has not been straight down. I cleaned his man bits and removed a good amount of smeg etc. Had a good feel. No bean. Nothing odd up there. Thought that might be why he hasn’t withdrawn his Willy up or down. This morning, the rug was soaked again. I’ve got his teeth being done next week so can discuss with my vet. I’ve rung my vet today for a chat but no one available. His behaviour is the same. Happy happy boy plodding around. But it’s not normal to wet yourself. I wish I cooks catch him having a pee to see what’s happening. 1937E131-40EE-4224-8C27-6BDBC4B478CF.jpeg

If his sheath is very swollen he might not be able to let down to pee. Lots of old geldings with Cushing's can get like this. Is his level of medication correct, when we're his levels last checked? Bute is sometimes prescribed to reduce the swelling. It worked for my old pony when his sheath was swollen.
If his sweet itch rug has a belly part make sure it is tight to his belly at the back.

Robin never drops his, the the very tip comes out only and there is constantly black crud in front of his sheath. His sheath is always disgusting. I clean it regularly (weekly) and vet has had a good look each time he has been sedated. No bean or any issue. No swelling tho. There was a gelding with a HUGE sheath at work. He was elderly with cushings. The first time I saw it I dashed off to tell someone. Turns out it has been like that for years and to ignore it.

I’ll ask the lady at his old yard when he was last checked. Bloods etc. I check
The rug and adjust the straps.

his sheath is very big too. Why would Cushings cause the swelling though ?

I think it was lack of circulation the reason given. This horse had prasend increased as cushings advanced but made no difference. It was monitored but nothing needed done.

My gelding who doesn't have any issues with his sheath etc does sometimes manage to pee over the belly panel of his sweetitch rug . Maybe the belly covering area has slipped back?

In some, cushings can also cause increased androgen production due to the wonky pituitary hormone ACTH signalling, so in geldings this could enlarge the sheath, cause more hairyness/late shedding.
