Grunt noise after a day grazing

A bit of a strange one. Horse has been turned out very slowly over a period of two weeks after a long period in 24/7. There is also v little grass due to no rain. He seems fine in himself but occasionally makes a grunt noise and clenches as if he is going to do some droppings. He may do it a couple of times and then stops, eats, poos, acts normally.

Any suggestions? He does have Protexin Gut Balancer and we occasionally give him the paste which seems to help. Has anyone experienced similar?

Slight gut spasm perhaps? Gas moving through?
Friends horse grunts and groans dreadfully after any time on newer grass. It takes him a week or two to adjust. First few times he did it, friend was terrified.
Protexin gut balancer is definitely a good move.
