Head tilting on the lunge! Help Needed

I have a 13'2 Welsh Section C Gelding, hes 7 years old and has been on and off uncomfortable for around 6 months now. He started head tilting to the outside and grunting on the lunge and was extremely stuffy. I had his back treated one a week for 6 weeks, took him to the vet and they nerve blocked him and found nothing. His teeth have been done and he is in good health. He started to improve and so I carried on working him, now 4 months on hes started doing it again. He isnt lame as such just stuffy and he does it on both reins more so on the right rein. Any ideas???
 
What is he like when you ride him? Were sore spots only found along the back?

There are so many avenues you could explore, but I would certainly get your Vets opinion again, you obviously feel he is not "right".

I presume he still head tilts when lunging with only a cavesson on.

It sounds as if there is something going on that he is in general seeming to cope with but is causing him some degree of pain/ discomfort, my guess would be his feet and think the stuffiness, head tilting and grunting would make me think low grade laminitis, possibly with some degree of rotation that is not being picked up by the vet for some reason, as he has not responded to nerve blocks, although you do not say what part of him was blocked, I would still ask for front feet xrays to be done anyway even if it just rules out the most likely cause.

If your vet is not really on the ball ask for a referral as it is a long while for him to be uncomfortable and you really need to get to the bottom of it, in the meantime I would look at his diet and treat him as a laminitic even if he is not actually overweight or showing other symptoms.
