If you are struggling and feel you can't cope, and your horse is fine when turned out, I would simply turn away until autumn. You may well find the problem has resolved by then.

You may feel more resilient to investigate the issue next year.

If that isn't an option for you, what investigations has the vet actually done? I can't see that the vet has visited? I would start with that.
