Help please, swollen dangly bits, with photo.

A friends pony came in from the field with a swollen penis. It was dripping blood, seemingly from inside as no evidence of cuts or damage from the outside, and he was in a lot of pain and discomfort.

He is on anti biotics, anti inflamatories and painkillers but the swelling doesn't seem to be going down. He is happy in himself now - probably not surprising with the meds he is on - but if anything the swelling seems worse.

The vet seems to think it was caused by a kick or a sting, but no external evidence of either.

Has anyone else seen something like this:

Vanner, he can indeed wee alright. There was some concern initially, but we saw him wee yesterday, a nice steady strong stream. He hasn't been given a muscle relaxant but owner was considering asking vet to sedate him to let it all hang out and perhaps it disperse a little. As lionman suggested, owner feels that the sheath is acting as a band, so sedation might enable the swelling to move.
 
Ouchies! I suppose it's rather sore, but have you had a 'rummage' round up there? I've known a friends horse go like that when there was a lot of crud stuck up there and needed a good cleaning out :)

some times boys are never happy with their 'tinky winks' whether they have problems with them or not! :D

if the sheath is acting like a band keep an eye it isnt cutting off the circulation to the penis although doesnt appear to be from the pic. Id put some lubrication around and inside the sheath though to facilitate movement of the tissue as lionman already mentioned.
 
If there isnt any improvement in a few days, I would seek more investigations! Have your friends had a really good look/feel around the area, no hair or anything caught? Probably not in the right area of the county but would highly recommend Justin Perkins at RVC, who looked after my old boy when he had "Willy" problems, and really new his stuff! Hope the swelling goes down soon, and no long term problems.
 
Can't imagine its got anything to do with it, but your vet (or your friend) has checked for and removed any smegma bean that might have built up? Just something to exclude if it hasn't been thought about already which I'm sure it has
 
The only time we've had this is using a proper sheath cleaner. Pony had a reaction and whole thing was very swollen. It went down after a few days and he had no problem weeing. I'd also be inclined to moisten it a bit, though I dont know what with!
 
Baby Oil! That's what my vet said to use to clean em with. Just squirt it on and it frees up any crusty bits or muck. Sorry to be graffic but it works.
 
Most likely a sting of some sort - but getting that swelling to disperse is step 1. Anti-inflammatories and ABs will help but it's a hell of a bad area for clearing swelling! I would be cold hosing for 10 minutes at a time, at least 3 times a day, and while the hose is on the affected part, squeeze it gently with the tips of your fingers to push the swelling around and encourage dispersal. (The cold water will help - but it's main role is to numb the area a bit so he doesn't object too much to the squeezing!)

After hosing and squeezing, liberal application of good quality baby oil!
 
Not as bad as that but my lads sheath swelled up so much in the summer it looked like he had goolies :), think it was either a reaction to a insect bite or he some how manage to scratch it, was advised to try and sluice out with warm water and keep lubricated, not easy when you have a hoof aimed threateningly at your head everytime you bent down to look :)
 
ouch... not sure what one can suggest, but at least the vet has seen it and is giving some pain relief, hopefully the swelling will go down and things become normal again soon. Our little pony had a tick on his bits and pieces, the sheath was hugely swollen and must have been very uncomfortable, but relatively short term. 24 hours or so.
 
Omg...have seen something just like this! Used to work on a big yard with film horses and one got kicked in the Willy, it swelled up huge and he couldn't pull it back in. We had to make a sling for it, as hanging down all the time made it swell more, and it had to be massaged twice a day with baby oil! I got the mick taken out of me for weeks for it but the horse was ok lol!
 
The vet should be able to give some sort of relaxant that will let it all hang out till the swelling goes down

As an aside my horse had something similar and cleaning the sheath at the time made things so much worse and aggravated it
