Help please, swollen sheath/dangly bits. With Photo.

A friends pony came in from the field with a swollen penis. It was dripping blood, seemingly from inside as no evidence of cuts or damage from the outside, and he was in a lot of pain and discomfort.

He is on anti biotics, anti inflamatories and painkillers but the swelling doesn't seem to be going down. He is happy in himself now - probably not surprising with the meds he is on - but if anything the swelling seems worse.

The vet seems to think it was caused by a kick or a sting, but no external evidence of either.

Has anyone else seen something like this:

Ouch!! Poor lad. Can't say I've seen anything in person quite like it, but did see a rescued WHW case with something similar. I 'think' it was nerve damage, and it did get better, but not entirely normal. As long as he is pain free, and can do a wee, I guess it will be a waiting game.
Poor lad again, ouch!
 
Our yard manager's horse once had something like this and it turned out to be a piece of kidney stone that had broken off and got stuck in the urethra. Ow :-S I think in the end they used ultrasound to break it up further so he could wee it out.

Is the swelling on one side or all the way around?
