Help - Possible lost passport problem

I have moved house and my belongings have been in store for some time. I seem to have lost the file with all my horse's documentation in it including his passport and I can't remember which company I used to obtain the original about 12 years ago.

The original came in a clear plastic folder and was printed on yellow card/paper. Does anyone have one like this and can you tell me who the issuers are so I can contact them about a copy?

Both of my horses are passported with the Passport Agency. Both have yellow pages and one has clear plastic, the other has a clear front, red back.... Hope this helps...
