It looks like shes torn while foaling in the past and its not been repaired and been left to heal by itself. It looks like she may have a hole but its not clear. Id get a vet to look at her and see what they say, they may be able to tidy it up by doing a partial caslicks procedure on her.

It's basically got a chunk missing so there is a constant hole, does this make her open to infection ect, the vet Is comin next week, she's Ovs lived with this for a while as it is well healed ect

Id say shes torn herself foaling, She'd possibly be more susceptable to uterine infections during her seasons when her cervix is open. Its hard to tell from the photo though, if the hole opens into her vagina then its more likely as faeces can pass into her vagina and cause a bacterial contamination. I'd let the vet look at it and see what they think though it looks like shes managed with it so far no not urgent!
