I've just gone through exactly this with my 16yo gelding. Initial lameness was presenting in front - xrays clear but nerve blocked and scanned and found to be his suspensory. No actual lesion, just strained. Physio and vet had both suspected hocks for a while so both xrayed and mild OA starting in both and were very likely the cause of the suspensory strain. We treated the suspensory first and lightly rehabbed and then treated the hocks with Arthramed a few months later. Had been sound all that time however when rehabbing from hocks he became not quite right in front again (mildly but definitely a backwards step to where we had been). A week of bute, dropping the rehab right back down to basics and slowing everything down sorted him out. Physio agreed it was likely just a bit too much too soon (I certainly wasn't hammering by any stretch but introducing trot too soon or lengthening sessions too soon etc) and with lots of exercises from her and vets approval he is so much stronger again and (touch wood) he is feeling absolutely great now. Our ultimate goal is to build up his back end as best we can to "protect" his front end.

Mine had undiagnosed front lameness on and off whilst lame with hocks and SI.
Bone scan showed hot spots in both knees and shoulders.
But x-rays showed nothing.
So it was deemed compensatory.
Now that he's sound with hocks the front end has been fine too.
