Horse lame after SI injection

My horse had SI injection a month ago for minor SI issue, 4 days later he started to get very sore over quarters, stiffness progressed to the point when he comes out of the stable he looks terrible (even if only been standing for couple of hours), but it pretty much walks off after 5 or 10 minutes. Has anyone else experienced this?
 
What has the vet and or physio said? my horse had his SI injected and was turned out as much as possible afterwards usually coming in first thing in the morning, being ridden, staying in for the rest of the day with a second exercise/ physio session during the early afternoon, he was never sore or stiff and responded better than expected.
I would be concerned that there may be an infection in the area for it to have become sore within 4 days and to be looking "terrible" after a couple of hours in, there is a slight risk of infection being introduced with any injection however carefully they prepare the area, or getting in to the site post injection which is why they have to stay in for a day or two afterwards.
 
Well I guess we ruled out infection now as its 9 weeks and he's still terrible, likely that he'll go for a bone scan :(
The vet and my physio (both fab reputations) are confused by his symptoms.
