Generally going ok. Perfectly acceptable to have a wee over night/when left, sometimes a poo too, but that is getting less so. All good. Happy.
BUT, on the odd occassion, which is sadly becoming more common, is that pup will do a wee when we are there, and when the door is open, often soon after doing one in the right place outside, and got praise for it.
So, twice now, mid-wee, I've picked her up and taken her outside. (think this is the right thing to do?) BUT when I scoop her up, she screams. I'm hoping this is because I've taken her by surprise, but I do say 'No' when I see her crouch. I'm sure I'm not hurting her, but what if I am? I'm getting worried!

she may just be a bit frightened when you say no, so try just picking her up and putting outside without saying anything and praise as soon as she wees outside. maybe when she wees in the garden she gets distracted and doesn't do as much as she needs to so maybe keep her outside for longer....good luck

Avoid ammonia based products to clean up any accidents as they smell like wee to a dog. I have found white vinegar effective. The key to puppy training is to keep it all relaxed. A startled pup' s brain is 'flooded' and unable to make connections. A little game when outside can lead to a happy pup relieving herself fully as well, as the exercise will stimulate her to go. Good luck from me too, don't worry, setbacks in toilet training have happened to most of us, it is sometimes hard to fathom what is going through a pup' s head!

If you spend some time really watching her behaviour you will see that she give earlier indicators that she needs to go than just going, you really need to step in at that point-it will only take a few times and the habit of going indoors is generally broken.
many young dogs will not go outside to toilet if their humans are indoors as it means leaving the security of those that make them feel safe.
If you spook her when she is going you will end up with a dog that becomes secretive and will not 'go' in front of you-I had one many years ago that would not toilet in the garden or on the lead but would hide it in corners of the house and behind furniture-that took a very long time to put right.

As twiggy says, if you say No when she starts to wee, she will think she is doing wrong and start going behind furniture etc. She is still at the stage where you need to watch her constantly, and take her outside every time she shows signs (and stay out with her). I would suggest you pm CAYLA and ask her to send you a copy of her puppy training guide (and perhaps make a small donation to her rescue) , it really is excellent.
