My horse has never had the nicest of canters she looks awkward and it has been a bit jerky not so you could see from the ground but without stirrups you could feel it. She has since been on box rest for 9 months and now when cantering her it is extremely jerky, her neck sticks out and every stride jerks you out of the saddle and it looks terrible from the ground but as soon as I get off her canter is smooth and elegant again. What can I do?

sounds like shes unbalanced, if back, teeth, tack etc all ok? Try work to increase strength, i.e. hill work, trotting poles. Just do short bursts of canter to start with, lots of transitions etc. I find walk - canter transitions whilst out hacking quite helpful.
