I posted last year about my horse and his terrible wither rubs that he suffers with, last winter it got really sore. This year I’ve all but managed to keep on top of things.
It only takes a short time in the wrong rug though to start rubbing. It’s bald but not inflamed looking. I left him a few times in his thermatex type rug after being ridden and that made it bald ( it was thin hair but definitely a light covering)
So today instead of that I used a waffle, nice and light surely… no its worse again.
Is there such thing as a wug/ high neck style cooler that might work? Most full neck coolers have the seem as a in normal neck rug then with a attached neck, if that makes sense!!

Weatherbeeta do one with a double fleece rolled neck/collar which comes quite high and PE do one where the neck opening is quite small (a bit like a Rambo) so the top part is well forward of the withers.

You could try sewing a sponge to the inside of the rug across his withers. I did this with a horse once and it worked really well.
I have tried that in the past, I think I needed sturdier sponge!
I do have a plan in place as I’d love to try and get the hair back this summer, he does need fly rugs which also rub though 😫
I’m going to try a bib with an inflatable dog collar stitched in. 🤞 super impressed with that idea 😂😂

That’s a good shout. I have a similar version of the first one you mention already, never tried it as it’s my other horses and a size too small but may be worth a try.

There is a company that made cutaway wither rugs for Arab's so the mane didn't get rubbed, I think they were called Arabian emporium not sure if they still make them though might be worth googling as might be others that make them now.
They sound perfect but sadly no they no longer exist. I did read some great reviews on an Arab forum in my search though!

Such a shame because they were really nice rugs I bet you can get them in the US they are really into showing Arab's over there.

kramer do. The one I was thinking of is brilliant like chamois leather material and works so well, but they do actual fleece as well. If you have something about 16.1hh TB size I've got one of the chamois leather super soft with a neck dry rugs you could have. Its in storage at my friends but I wont need it again and its sad for it not to be used!

I’ve tried so many padding each side, when the rug is on top most things seems to compress or slide.
It’s brain numbingly annoying.
I’ve manage to keep from being properly sore this year with constant rug changing, if he’s in I absolutely have to remove turnouts to give time in a light shiny stable rug.
Wet rugs get changed as soon as possible and a light shiny combo under any other rugs where possible has worked very well.
As are his crappy Mark Todd turnouts, shame
They are not better quality as the shape is perfect for him
I’m Dun, he’s 16.1/2 chunkier than a TB but not a massive horse. Usually 6’6 or 6’9 in rugs depending on make.
Worth a try?

Sew in some medical grade sheepskin. You can get squares of it off Amazon. Used for pressure relief and bedsores. Works a treat for sewing into rugs and also as a girth pad.
