I think you need to have this conversation with your farrier and your vet to see what they think.
Each horse is different and all horses with spavin may need to be shod differently according to their confirmation, the way they stand and the way they move.
If the horse is placing its foot midline or it twists its foot to reduce pain, then lateral extensions will encourage correct foot placement. Sometimes egg bar shoes will help by providing heel support and by your farrier correctly balancing the hind feet stress on the hock joint will be reduced. Shoes with square toes can assist breakover and help to reduce pain.
