Hope this is of use.

age she's now 10 rising 11 but got laminitis aged 7
british riding pony 13.2hh
used as a leisure / show pony
she has been 4 years clear with careful management year round
her body score is presently 4, as been out of work due to no where to ride
this is monitored weekly
she has been known to crib and gets stressed easily
the year she got laminitis she got beaten up by a rig in the field it almost killed her 2 weeks later she went crippled lame with laminitis.
she is turned out at 9am and back in at 3pm in summer she is restricted with use of a green graze muzzle and also restricted on frozen grass in winter i try to avoid turn out on this.
she is excercised upto 1 hour a day maybe more now we have somewhere to ride
she is stabled out of sight of other as she stresses when she sees them turning out or coming in and ever since she was beaten up she stresses if another horse invades her space.
she has a horse ball and lickets in her stable which measures 10x12
bedding is shavings so she can't eat it
her water is changed daily more if she is stabled or it is hot or cold
she has no after effects of the one dose she has had but that is not to say i don't still monitor and manage her closely at all times

I put my answers in bold:
A friend of mine is at uni, and a part of her work is to get as many people to fill in this Laminitis survey as poss.

HORSE ONE: TB, 11yo now, first got lami as 8yo, 15hh

4. use: leisure/competition

5. what is the horses laminitis history?
prone to laminitis - very very very sensitive once had got it the first time - until then had been fine on any grass and any feed.

6. body condition score:
No idea - you can feel her ribs but not see them, she's shiny and healthy.

7. Is your horses weight/ body condition score monitored regularly?
yes, weekly - every time I groom her (so daily), as she drops weight v fast and we have to be careful.

8. Does your horse perform any stereotypical behaviors?
No unless she's stressed 'cause she can't see her friends, then she box-walks.

9. Which of the following best describes the management system of your horse throughout the year?
stabled with a few hours turnout daily and lots of exercise.

Laminitis part 2

1. How many hours per day is your horse turned out in the field (can include turnout on dry lots)?
Depends on season (obviously!). Up to an hour in bright sunlight and warm weather, 4-6 hours in cool temps (above freezing up to 10 degrees or so) unmuzzled, longer muzzled. We take it as it comes and she gets as much as poss depending on the grass. If it's frosty all day she can't go out at all.

2. what type of turnout does your horse have access too?
full field (same as other horses) - but our grass is v v poor lol!

3. Is the horses access to turnout altered during times when non structural carbohydrates (sugars) may be higher in grass (spring, early summer, autumn)
as above

4. During turnout is your horse in company of other horses or isolated?
with other horse/ horses

5. How long is your horse exercised for per day?
(can include time spent on walkers, hacking, etc)
0-1 hours if getting a fair bit of turnout
1-2 hours if turnout an hour only.
2-3 hours if no turnout at all.

6. How many rations of forage are fed per day?
Don't know what a 'ration' is - she gets two slices of horsehage per day, and a slice of oat straw. She never runs out of forage. She can't have hay though, even soaked she can only have a little bit, only straw or timothy horsehage - anything else sets her lami off.

7. During stabling how much contact with other horses does your horse have?
is in sight of other horses

8. During long periods of stabling are any forms of enrichment provided?
??? Plenty of exercise (twice daily), constant forage supply, friend tied up outside her stable for an hour or so for company when I'm there, some of that exercise will also be with a friend with time given to groom etc, tied up outside to eat when I'm there so she's outside, and she eats her dinner with the other horses, across the fence from them. She doesn't play with 'toys', and can't have lickits etc.

9. What size is your horses stable?

12 x 12 ft

10. During stabling what type of bedding is used?
shavings

Laminitis part 3

1. How often is the horse supplied with clean water each day?
has continuous access (automatic water feeders, troughs etc) - large (enormous) tub trug which is checked twice daily, topped up, never ever runs out (would supply 3 horses easily), and is emptied and completely refilled every few days or if she drops stuff in it and fouls the water.

2. Does your horse appear to be in any physical discomfort? (whether laminitis or any other physical problems, injury etc)
no

3. Is your horse currently receiving any form of pain management
for laminitis or any other physical problems?
no not at all

HORSES TWO, THREE, FOUR
- Haven't got time to do this long thingy again but none have any stereotypical behaviours and:
Horse two: never had lami, 10yo NFx, lives out with hay in winter, worked 3-5 days a week for an hour or so. Body condition can feel ribs but not see.
Horse three: never had lami, 5yo WBx lives out with hay in winter - worked 3x week in winter, 5x week in summer, 1-2 hrs. Body condition can feel ribs but not see.
Horse four: had a little lami when I got her, 6yo WBxWelsh, lives out with hay in winter, restricted grazing (still living out) in summer, worked daily in summer for hour, 3x week in winter for hour. Body condition can feel ribs but not see.
