Medial refers to the middle and lateral refers to the side. A medial lateral foot imbalance can cause pressure within the hoof causing arthritis and bruising.

Sorry to make it so simple but, are you sure it is the med (inside) of the foot that is taking the weight, and if it is could it be wearing due to action not weight load?

I've seen this type of imbalance on very narrow chested horses, due to their action.
