Medical Armband Insert - Help!

Help! I'm taking part in a hunter trial on Sunday for the first time in years and realise I will probably need a medical armband. Due to the postal workers strike, I'm unlikely to get one that needs to be posted and my local saddlery only sells the armband without the medical insert, which seems slightly bizarre! Does anyone know where I can download a medical armband insert in pdf format so I can print one off? I can only find downloads for American and Irish riders and British Eventing doesn't have one available in a downloadable format!

If you get really stuck just make your own up
I know it's not correct but it's better than nothing
Include name, medical history, date of last tetanus, contact numbers etc

I could try scanning one in for you, if that's any use? Or could just copy type it onto here. You need to include doctor's name and address, any known allergies etc.

I lost the insert in one of mine and just wrote down the basics including contact telephone numbers. No-one checks them, btw!

Can you ring BE and see if they can email you a template of the info you need to put in it?

I did ring BE and they said they had nothing they could send me, other than if I bought one and it was sent by post, which doesn't help when there's a postal strike and it's needed in 3 days' time! Not the most helpful reply I've ever had!

I have put a large sticker across mine saying "NOT TO BE TREATED BY ST JOHN'S AMBULANCE - VET OR REAL MEDIC ONLY"
