Medical armbands- what info is needed?

What needs to be on the card? I need to get a Medical armband but I have a slot for one on my body protector so just need to make a card.

Name
DOB
Bloodgroup
Emergency contact no.
Vehicle Registration no.
Doctors name

Is there anything here I dont need or anything else I do need? It is just for an unaffiliated HT.

If you are allergic to anything that information might be useful.

Allergies..

Medical issues (asthma, angina, high blood pressure)

Any serious ops you may have had (spinal surgery.)

Put your blood group on but they wont use it.. unless its a rare type.. they will still crossmatch.
