medical armbands

I bought a medical armband but with no insert. . .how annoying.

Was told to make one. . .what details do i need to put on it? or does anyone know of one i can download? i found one on http://www.airc.ie/index.php?option=com_wrapper&Itemid=71 but it confussed me because there are two sheets?

in the following order:
PAGE 1
name of gp
address of gp
tel no
riders name
dob
religion
permenant address
tel no
(then leave a few lines for change of address)

PAGE 2/3
It then asks you to list any previous injuries, surgical ops or medical conditions

PAGE 4/5
Then there's a blank page for any medical officer's suspension reports
Then a blank page for when clearance of any suspensions is given

Final page 6: next of kin details (name, relationship, address, tel no & mobile no.
Lastly horse box/trailer details - make, colour, reg.

There's also a couple of pages of literature in there concerning rules etc. but nothing for you to fill in on those.

If you pm me with your email address, I can send you a template of the medical card that came with my British Riding Clubs armband. I lent the armband to someone but took a copy of the insert before anything got filled in.

I've sent it but it will take a while to come through as it is a big file. I have a simpler one that came in a free armband I got from Good Year, I will send that as well in another email.
