Mystery lameness

Here is the report from my recent trip to the vet, he is now on day 17 of box rest and it seems to be getting worse. Am so worried, no heat or swelling, negative to hoof testers. Hoping someone had a similar situation and can offer some advice or encouraging words !

VETERINARIAN REPORT

Fudge presented to WestVETS Equine Hospital for evaluation of a right forelimb lameness that had been present for several weeks. On examination, the horse had an inconsistent right forelimb lameness that worsens with exercise. The lameness was graded 2/5 on the right front. Lunging exacerbated the lameness on the right front on the right circle. Flexion did not worsen the lameness. The lameness locator was used on Fudge which agreed with the right front lameness.

Diagnostic analgesia: Blocks that were performed on the right front limb are as follows Palmar digital: No change

. Abaxial sesamoid: No change

. Low 4 point: No change

High 4 point: No change

Medial ulnar: No change

Shoulder joint: No change

The following day, the abaxial nerve block was performed and it was still negative

Diagnostic Imaging

. Radiographs were performed on both front hooves, the right front fetlock, right knee, right elbow, right shoulder and neck.

• There were no significant findings on the radiographs.

Recommendations:

1. Stall rest/very small paddock confinement for 4 weeks.

2. Recheck in 3-4 weeks 3. If there is still no improvement, Fudge would be candidate for a bone scan

So they x-rayed joints but not the bones the connect them? Strange. Did they scan any soft tissue? I know it's hard for the DDFT in the hoof but they wouldn't also necessarily come sound from a nerve block there.

They xrayed joints and bones - they didn’t scan as when they blocked the whole leg he was still lame so they said it couldn’t be soft tissue/tendon etc which I totally don’t understand- either it’s soft tissue or tendon etc in that leg ( as all X-rays clean) or it’s coming from somewhere else - but they are adamant their lameness locator confirms it’s in that leg ??‍♀️ so confused !

I knew a horse years ago with very similar wrote up/symptoms. He turned out to have nerve damage in his shoulder. Poss from a kick/fall/bang in stable
