Hi all,
Have a 15.2 cob type 4 year old have is intermittently lame and seem to be sound at the start and become lame after exercise. Had the vet out and she said she mostly likely had a hoof imbalance, got the farrier out who said he’d fixed the imbalance however she is still lame after hard work although a little better. She is very fat and greedy (working on slimming her down!) so was thinking it could be early laminitis? When farrier came a second type he said the foot had gone unbalanced again, but there was only 5 weeks between trims and no work on hard ground. She is nice and forward when ridden no reluctance to go forward. Barefoot. Any idea want it may be/ what to do next?

Barefeet growing wonky (and doing so repeatedly) can often be doing so in a response to issues higher up.
I’d be getting a full work up, worst post work tends to be soft tissue (we had a suspensory that only showed up the day after schooling on a surface- we didn’t have a surface so took longer than ideal to work that out)

Which leg/legs is it? Front or behind? Just one?
how long has it been going on for?
Next step would be nerve blocks with a decent vet to figure out the source

Hi, she’s lame on her front left only, when trotted up she doesn’t really dish but you can see that the leg looks twisted inwards

I’d be asking for X-rays and a farrier to work with the vet. It may be a conformation fault that you simply can’t fix, so be prepared to hear that.
