Nodding Pony

My friends daughters pony is nodding in trot, it isn't consistently though.

He has been checked for abscesses, lameness etc but is fine. He was stiff in his neck which has been seen to by the physio. A few months ago he had a small puncture wound in his frog from a stone and an abscess (not at the same time) only thing we can think of is that he has it in his head that its painful when there's nothing there. The farrier said he does have a bit of side bone but that it shouldn't affect him.

What else could it be? Would it be worth getting the vet or physio back out? Or should they get the back person out?
 
Is it just when being ridden or inhand? If its when ridden it may be that he is unbalanced. Apparantly its called bridle lameness :/
 
Teeth, back and saddle checked would be my first check. My mare had quirks like this and found sharp teeth leading to ulceration and a sore back combined with poor feet.
 
Its ridden and inhand but more ridden i think. He is 19 now.

The vet is booked for Friday but will also get his teeth and back done. His teeth were.done before they had him so he was booked in for July but will bring it forward
