I find sudocreme is really good for stopping midges.
 
CD&M Killitch for the midges /wrinkles....leaves a nice silky skin surface, and definitely clobbers the midges.

A low profile girth like this is definitely worth a try, though if the Stubben string girth hasn't worked (similarly soft and low profile) then this may not.

I would say that girth rubs are more common with horses whose thoracic sling has dropped through their shoulders, it leaves very little space for a girth in some cases, but it does sound like the rug and flies are the main fix, but I'd certainly consider posture. It's likely at least part of the explanation for Sharon May Davis' finding from her dissections that every single ridden horse she's ever worked on had damaged elbows, less so driving horses so it's probably TS plus the loading of a rider.
