Pain behind the ears causing headshaking- any ideas?

A horse in our yard has been showing very strange signs of headshaking. He can go for weeks at a time in all weathers and circumstances without any signs then all of a sudden becomes a headshaker. We have had the vet to him but they didn't really understand what I was saying. We have since had the chiropractor who said he felt he was fine and then I have had the physio who said there was a huge difference in the right side to the left, the left being tense and tight and he was perfect to do until she tried to do behind his left ear where he got upset and started shaking his head. It would be where the headpiece sits.
Has anyone had any experience of this- its not when you touch his ears but when you touch about an inch behind them. He is fine to trim out his ears, tack up ect. He has had his teeth done.
Before the headshaking comes on he tilts his head to the left for a few days prior to this. There are no marks or signs of trauma.

jayjay did this and it turns out the head piece was too tight on him causing pressure behind his ears

It is amazingly common to see headshaking caused by the browband being too tight and pulling the headpiece. Might be wrth just trying a slightly bigger one for a while.
