Can you get a passport completely changed?

I have a passport for my little boy which has several errors in it....eg it says his mane is black when it is white, and it says his expected height at maturity is 13.2 when he is 14.3 now (he is 3)....

It is a 'non-registered equine' passport that contains no information apart from the day he was foaled (which could be made up anyway!) and the 2 shaded pictures of him....

Not that it *really* matters, but I would like his passport to be relevant to him!!!

....that got me to wondering what happens if you lose a passport, but havent got a clue who issued it in the first place, and have no record of the passport number?? Can you get a new one in that situation?
 
I had my boy's passport updated as it said he was a liver chestnut colt and he had become a grey gelding! Also I needed to update the address, so I phoned the issuer and for a fee they amended everything and sent it back quickly. If you don't know the issuer I would phone the BHS for advice?
