Occasionally puncture wounds can result in infection of the pedal bone (septic pedal osteitis). The infected area
of bone needs to be removed until healthy bone is reached. This treatment may be carried out under local anaesthetic, but sometimes it requires a general anaesthetic. Your horse may need to be referred to a specialist equine hospital for this to be done. If treated early, the prognosis can be
reasonable.

It usually occurs secondary to a puncture wound and if they are treated early then the prognosis is good. The treatment involves removing the penetrated area of sole and curetting the underlying tissues and bone to remove the infected area (called a Streetnail procedure). The aftercare is pretty involved as the site must be protected. Sometimes a special shoe is put on with a sole plate that can be screwed on and off; other times the foot is packed with swabs and bandaged. As you can imagine they are pretty painful after surgery.
Some vets will do it under standing sedation, others under GA, but either way it is a fairly specialist job.
Hope that is useful. If your horse is suffering from this then get your vet to explain what is going on- how extensiuve the bony damage is etc, and ask to see the xrays. Generally the prognosis is good so hopefully he/she will make a full recovery.
My vet 'mentioned it in passing' last week when my old lady had yet another visit. He didn't think she had it but would investigate if her hoof did not show any signs of improvement - which she hasn't. for her it would mean PTS.
Just read some of your other posts and I'm sorry to hear she's no better. Might be worth having her foot Xrayed to see if this is the case. Hoof abscesses can linger for a long time, particularly if a horse is a bit run down, and I know how frustrationg it can be. Hopefully you'll get to the bottom of it soon and the outcome will be positive.
Thankyou. It may not be ps, but next step is another chat with the vet. Was just wondering about this. She is very old and underweight with I suspect a low immune system. I just hate seeing her in continuing pain
