Permanent loan contract?

Having only ever loaned out a pony once a few years ago and only short term, I'm now offering one of my horses on a permanent loan basis (I would sell him but want to ensure he has a happy future as he's 15).

I'd really appreciate ideas of what should be written into the contract?

Also, I know passport-wise it has to go with the horse but what's to stop them changing ownership in the passport?! Anyone can forge a signature and send it off... What proof would I then have that he's mine?
 
I think the contract would be the same as any other loan contract (see BHS?), just without dates for return - a loan is a loan, there is still the option to take back/return..

Re the passport, perhaps write "this pony is on loan to XXXXX from yy/yy/yyyy until this passport is signed otherwise. This pony should never be sold. Write it on the back of a page that is important, such as markings? Sign it yourself and get them to sign it, labelling underneath, owner and loaner?
