I know there are people that are for/against this type of training aid, but for those of you that use them, can anyone tell me how tight/loose it should be fitted?

I am only using it on the lowest setting (clip between horses legs to underside of roller) but its not made clear on the instructions how much slack there should be in the ropes and pully's and exactly where the sheepskin bit behind the legs should hang.

The sheepskin bit should be as high up as possible really, so right at the top of the hind legs. Any lower and the horse gets jabbed in the mouth eveytime it moves its hind legs!! I have mine relatively tight to encourage, but not hold him in. It should come into action when they dont work correctly but not pull them into an outline if that makes sense!?
 
when i got mine they recommend the back sheepskin piece is at stifle level at its lowest point, use it on a setting that allows the horse to stretch down and vary its head position as they become uncomfortable if held in one position too long.
