Reaction to being injected?!

Has anyone else experienced this?

Back story: vet came out to vaccinate before Christmas. 48 hours later the little mare was beside herself itching and comfortable. Her glands were rock hard, the injection site swelled and her whole neck swelled. She had fluid collecting under her stomach and just took really badly to it. Vet came out at said it was a reaction to the vaccine. Gave her some steroids and left. It did take a good ten days to clear up and she was still left with some scabby patches where the skin was so irritated it broke.

Fast forward to last week, we took her to the vets for a dental clinic. You cannot get an IV in this horse for love nor money so they sedated her via an IM line. All good. However yesterday evening we noticed the same thing... rock hard glands, thick neck, starting to get itchy. So my concern is that it wasn’t the vaccine that she reacted to, it’s the act of being injected? Has anyone else had this happen or know what causes it?

Not a vaccine, but one of my mares was given injectable ABs after an operation , and after the first one into her chest, a huge swelling came up between her front legs, half has gone down after 1 week, other is a bit smaller. Apparently it's a fairly normal reaction
 
I believe it’s a reaction to the carrier fluid...my mares experienced it once and now the vets know they select drugs with an appropriate carrier.
