Hiya, my welshie has just been rescoped after 4 weeks of injectable omeprazole (7 days apart) and sucralfate powder in his feed (9g twice daily). He had grade 2 squamous ulcers and mild hyperaemic areas in the glandular region on his original scope. This time his squamous region is totally clear 🥳 but the areas in his glandular region look more inflamed than before. We have decided to do another 3 omeprazole injection (first one directly after his scope) 5 days apart this time, and continue with sucralfate as before. Has anyone experienced this? He is getting hay in the field, turned out 24/7 with a friend, fed small amounts of speedibeet and thunderbrooks chaff with pectin, lecithin and magnesium hydroxide. He also has protexin gut balancer which he has been on the whole 11 months I've had him. He has not been in work during treatment
 
Glandular ones are generally the harder type to treat sadly. What is the cause? Assume he's had a workup and everything checked? Teeth, Saddle, etc?

We believe the cause to be stress, he's had a rough time in the year before coming to me and was very reactive and stressed out. Only had him about 7 months before we looked into ulcers due to his behaviour and past. Serious separation anxiety issues, to the point that in the home before me he dropped weight in the first couple weeks there very quickly as he just wouldn't eat! He is far better now than when I got him and has also chilled out more in the last couple weeks so think the treatment has been helping 🤞he's had dentist, Saddle fittter, willy washer and physio (who works with my vets and suggested it was an ulcer issue not a lameness issue) in the 2 months before diagnosis. I am definitely keeping mind open to there being other issues he isn't showing but at this point vet isn't eager to do workup as he does such minimal work and there has been no sign of lameness. Was only hacking for a couple months before I started to question if there was something going on to make him reactive, then we got the kicking up at stomach when in the school which sealed the deal of scoping. He would do serpentine and circles no bother out hacking hut just shut down in the school (school is a more stressful place for him)
 
Mine has also been treated recently for glandular ulcers. He did scope clear after 5 injections of omeprazole combined with sucralfate. My vet said she hadn't had the same success with misoprostol compared with the original research paper and her recommendation was not to choose this option. I'm now adding slippery elm and aloe vera juice to his feed. Despite scoping clear mine is still tense on the right rein so like yours there may be something else going on. He's also had blood tests (including testosterone), back xray, teeth, physio and saddle fitter, there's nothing obvious.

I haven't got any useful suggestions but your not alone with this problem. Hope the extra treatment gets yours sorted out.

Thank you. I have seen numerous people saying omeprazole isn't useful for the glandular ulcers so have been panicking a bit. But found actual papers last night that say with injectable omeprazole they do get good results. Fingers crossed it does the trick.
He's never appeared lame thankfully (or not so thankfully because it would be an obvious starting point) . He's had front shoes put on not long before his first scope as he was a little pottery when we were hacking, became very determined to walk on any and every grass verge he could find after being fairly unphased before. Reckon as he's never done much at all before me and then we've been doing lots of hacking on varied ground that he got a little footsore as he's not got particularly solid feet! Hopefully that may have been part of it if there is a pain aspect.
Have had a horrible past experience with ulcers so I panic. Started crying as soon as I saw the glandular ulcers come into view on his rescope! Fingers crossed his next rescope has a better result
